feat(entra): add MT.1260 - high-risk users should not be left unremediated in Entra ID Protection - #2609
Part of #2518 (first of the two checks; MT.1261 for medium risk follows in a separate PR). Under #2510.
Entra ID Protection flags users it thinks are compromised. The flag only clears when the risk is remediated (password reset) or an admin dismisses it or confirms the user safe. Until then it just sits there. MT.1260 reports users whose risk level is high and whose risk is still unremediated.
Reads GET /identityProtection/riskyUsers filtered on riskLevel eq 'high'. Read-only, never changes a user's risk state.
Treats both atRisk and confirmedCompromised as unremediated. Filtering on atRisk alone would miss users an admin confirmed compromised, which is also the state the confirmCompromised API leaves a user in. remediated, dismissed, confirmedSafe and none are not reported.
The risk state is filtered client-side and only riskLevel server-side, to keep the Graph filter to one property.
On overlap: MT.1253 checks that a Conditional Access policy requiring risk remediation exists. This checks the outcome, users actually left unremediated. The .md says this so they read as a pair, not a duplicate.
✅ Checks
My pull request adheres to the code style of this project.
My code requires changes to the documentation.
I have updated the documentation as required.
The build and unit tests pass after running /powershell/tests/pester.ps1 locally.
ℹ️ Additional Information
Testing
Unit tests in Test-MtEntraUnremediatedHighRiskUser.Tests.ps1 (7 passing): no risky users, an atRisk user, a confirmedCompromised user, only remediated/dismissed users, a mixed set (reports only the unremediated one), a 403 (skips NotAuthorized), and an unexpected error (thrown to the engine).
Live test on my tenant: marked a test user compromised with confirmCompromised. MT.1260 failed and listed it, along with a second user that was already atRisk:
Dismissed both, cleared the Graph cache, reran: passed.
Regenerated the golden fixtures (selection.json). NativeTestAttributes and RepositoryLayout tests pass.

Summary by CodeRabbit
New Features
Added a check that reports Entra ID users with high risk who remain at risk or are confirmed compromised. Users with handled risk states are excluded, and the check does not change risk state.
Findings are rated High when a user has a privileged active or PIM-eligible role; role details are included when available.
Documentation
Added guidance on interpreting and remediating the new check, including its scope and related checks.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.

Walkthrough
Adds MT.1260 to report high-risk Entra ID users in unremediated risk states. The check evaluates privileged roles when users qualify. The change also adds tests and documentation, updates selection fixtures, and revises an EIDSCA.AP14 severity example.

Summary
MT.1260 check and documentation tests/maester/entra/Test.MT.1260.ps1, tests/maester/entra/Test.MT.1260.md
The check queries high-risk users and reports those in atRisk or confirmedCompromised states. It handles authorization errors, evaluates privileged active and eligible roles, and raises severity to High when a reported user has a privileged role. The documentation describes the check and remediation steps.

Explanation
The title names the main change: adding MT.1260, a check that reports high-risk Entra ID users whose risk remains unremediated. It is specific, single-sentence, and matches the changeset (new test script, docs, unit tests, and golden fixture update). The 'feat(entra):' prefix is a conventional-commit style and does not affect relevance.

Explanation
The description follows the repository template. It includes the Description section with a linked issue reference (Part of #2518, under #2510), a detailed explanation of the check's behavior, the Checks list with items marked, and Additional Information with testing evidence, live-test screenshots, and fixture regeneration. The template's HTML comment placeholders were removed, and the 'Closes #' line was replaced with 'Part of #2518', which is a reasonable adaptation because the PR does not fully close that issue. MT.1260: Area logging-and-detection is defensible because the test checks follow-up on Identity Protection detections. identity-lifecycle and access-control do not fit better under the area order.
MT.1260: CISA.MS.AAD.2.2 checks the high-risk notification setting, a tenant-wide setting. This test lists unremediated high-risk users. They are different checks, so the different ResourceType (User vs Tenant) is correct.
MT.1260: ImplementationCost Low is acceptable when a handful of users is flagged. If investigating each flagged account typically needs the user or another team, Medium would be worth considering. Under the lower-value rule, Low stands.
MT.1260: Severity Medium fits a failure limited to a few flagged accounts. The runtime raise to High for privileged users is consistent with the rubric's reach rule.

The mocks return fixed high-risk fixtures without checking the request arguments. Removing or changing -Filter "riskLevel eq 'high'" would still pass these tests. The client-side filter checks only riskState, so the check could report active users with lower risk levels.

Bypass the Graph cache for the MT.1260 risk-state read.
Invoke-MtTest -Id MT.1260 is a supported standalone entrypoint. It does not clear the session Graph cache. After an earlier run caches a high-risk user, remediation followed by another run in the same session can reuse the old response and still report that user as unremediated. MT.1260 must evaluate the current riskState.

Update the `Invoke-MtGraphRequest` call in MT.1260 that reads
`identityProtection/riskyUsers` to bypass the Graph cache, ensuring the test
evaluates the current `riskState` on each run.
Add an invocation assertion for the server-side riskLevel filter.
The MT.1260 tests cannot detect a removed or changed riskLevel eq 'high' filter. Every mocked risky user has riskLevel'high', and no test checks the $Filter argument passed to Invoke-MtGraphRequest. If the filter were removed, the check would still receive only high-risk users in the mock and would pass every test. In production, Graph would return medium- and low-risk users too. The check only filters on riskState, so medium-risk atRisk users would be reported as high-risk failures, which contradicts the documented behavior.
Add an assertion that verifies the request. The Should -Invoke form below is for Pester 5. Place it in a Describe block that has the existing BeforeEach mocks.
Do not add a medium-risk fixture that expects it to be excluded. The check does not filter riskLevel on the client, so that test would fail. The server-side filter is the only guard for riskLevel, so the request assertion is the correct test.

The Invoke-MtGraphRequest mock returns fixed data without checking its arguments. The suite could pass if production removed or changed -Filter "riskLevel eq 'high'".
The consumer filters only riskState. A non-high-risk user with atRisk or confirmedCompromised could then be reported.

Thanks @Dfrank77, this is a useful check, and raising severity for privileged users is a nice touch.
I pushed a commit on top of yours so we can merge:
Added Control = ('nist-800-53-r5:AC-2(12)', 'nist-800-53-r5:IR-4').
riskyUsers only accepts IdentityRiskyUser.Read.All, and Maester didn't request it, so I added it to the preview scopes (Connect-Maester -IncludePreview), listed it in the permissions docs and noted it in the guide.
Restored the comment-based help block, which 272d269 removed. It feeds the command reference page.
Two small things for a later tidy-up, not blocking:
The 403 check matches any message containing "authorization". 403|Forbidden|Authorization_RequestDenied is tighter.
If riskLastUpdatedDateTime doesn't parse, .ToUniversalTime() runs on $null.
Really appreciate you adding the NIST controls and wiring up the permission. That's twice now you've stepped in and added what was needed to get a PR over the line, and I'm paying attention to the patterns so I get it right upfront next time. The comment-based help block feeding the docs page is one I won't miss again.
I'll clean up the 403 match and the null parse in a follow-up.
The reason will be displayed to describe this comment to others. Learn more.
Re-approving after the help-block fix. Thanks again @Dfrank77.
