This new version seems to fail to list App::Info::RDBMS::PostgreSQL as a prerequisite maybe?

This is using cpanm on MacOS. Here's the entire log output:

You have make /usr/bin/make
You have LWP 6.82

I do have DBD::Pg 3.20.0 currently installed on this system, which was installed the same way, using cpanm, and that didn't fail. I see that 3.20.0's Makefile.PL also requires App::Info::RDBMS::PostgreSQL. I don't recall ever installing that module, so I don't understand why 3.20.0 didn't fail in the same way.

Well, I've used cpanm to install DBD::Pg 3.20.1 just fine. I'm using a different version of cpanm though, and I'm not on macOS. You should open an issue. This isn't really the place to be reporting or diagnosing installation issues.
That said, the weird thing i'm seeing is the @inc in Can't locate App/Info/RDBMS/PostgreSQL.pm in @inc. Last time I checked, Perl uses @inc, not @inc, so I don't what's going on there.
I don't see any @inc in the emails I sent, only @inc.
Here's an attempt to install without cpanm.
***@***.*** ~/.cpanm/work/1777651175.45068/DBD-Pg-3.20.1 % perl Makefile.PL
Configuring DBD::Pg 3.20.1

You don't need to have App::Info::RDBMS::PostgreSQL installed. It's not a Perl module that gets installed anywhere. It's part of the DBD::Pg distribution and only used during the installation process. There's nothing about this that has changed in many years.
Thanks. Looking further into this, I find that in 3.20.0's Makefile.PL
my %seplist = (
MacOS => ':',
MSWin32 => '\\',
whereas in 3.20.1 we have
my %seplist = (
MacOS => ':',
darwin => ':',
MSWin32 => '\\',
On MacOSX $^O is darwin and the separator should be '/', not ':'. This is why in the log output above we see
@inc entries checked: t:lib
which should be t/lib

Version 3.20.1 has been tagged and uploaded to CPAN via pause. Mostly important bug fixes and improvements. No feature changes, so not a major version bump. From the Changes file:
Set imp_dbh->async_sth when a prepare is successfully sent from pg_st_prepare_statement
Don't execute "result auto-retrieve" code for a successful async prepare
Increment imp_dbh->prepare_number when success of an async prepare is detected in handle_old_async
[Rainer Weikusat, reported by Michael Kröll]
(Github issue Mixup with prepared statement handles #183, pull request Handle async prepare correctly in handle_old_async #184)
Fix hangup of queries like "COPY TO STDOUT" in async mode
[Ed Sabol, Alexander Gorlov]
(Github issue Hanging execution of queries like "COPY TO STDIN" in asynchronous mode. #98, pull request Fixed hangup of execution of queries like "COPY TO STDIN" in asynchronous mode. #101 and Redux: Fix hangup of execution of queries like "COPY TO STDOUT" in asynchronous mode (now with tests) #163)
Improve documentation about sslmode options
[Ed Sabol, reported by Alexander Karelas]
(Github issue POD suggests it's impossible to verify server-side SSL certificates #180, pull request Issue #180: Update sslmode documentation #186)
Drop ExtUtils::MakeMaker requirement back to 6.58, but recommend 7.64
[Greg Sabino Mullane, Ed Sabol, reported by Peter Tribble]
(Github issue Build fails on perl 5.34 #170)
Shorten path for test database to reduce OS errors
[Greg Sabino Mullane, Ed Sabol, reported by Gianni Ceccarelli and Peter Dyballa]
(Github issue Tests fail when run in a directory with a long name #78 and DBD::Pg cannot be tested on macOS (or Mac OS X) with MacPorts package manager because socket path is too long #185)
